免責同意書入学時に所定のDeclaration（免責事項確認書）に署名することにより、学生、両親または法的保護者は、以下のことに同意するものとする。 Tom Fielding Golf School の所有者、経営者、役員、従業員、並びに予約は取次ぎを実施した旅行会社は日本とオーストラリアの法律により法的責任を免れる事象に関しては一切責任を負わないものとする。学生、両親または法的保護者はTom Fielding Golf Schoolまたはそのプログラム運営の為の関連団体組織が所有、賃貸、運営、管理する場所において、また、学生の為に手配された宿泊場所や授業の一環の各種行事において起きた、または学生によって起こされた、学生の死亡、怪我、身体障害、損失、損傷、所有物の損害に関してはいかなる請求の申し立てもしないものとする。さらに、学生もしくは学生が18歳未満の場合は、両親及び法的保護者は、 Tom Fielding Golf Schoolやその代理人、もしくは認知した機関により組まれたプログラム（スポーツ、文化、社会的、レクリエ―ション、その他）において、またお客様が出席、参加する種々の活動の場において、学校で負った直接的または間接的な費用を支払うことに同意する.

IMPORTANT NOTES for the Summer golf camp:

***You are responsible for your own travel and medical insurance
*** Tom Fielding Golf School is not responsible for loss or damage of golf equipment and personal items

Payment procedures:
Option 1: Full payment can be made at the time of application.
Option 2: A deposit of 50% will secure your application for the golf camp, with the balance of payment to be made one (1) month in advance of the starting day of the golf camp.

TOM FIELDING GOLF SCHOOL GOLF CAMP WAIVER AND RELEASE OF LIABILITY READ BEFORE SIGNING
In consideration of being allowed to participate in any way in the Tom Fielding Golf School's 2020 Japan Summer Time Golf Camp program, related events and activities, the undersigned acknowledges, appreciates, and agrees that:

The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,
I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,
I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,
I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS the Tom Fielding Golf School, Japan. their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

BOOKING CONDITIONS & CANCELLATION POLICY.

キャンセル料(決定後の全取消)・・・30日前10%、20日前20%、14日前50%
(人数変更など)・・・7日前20%、前日50%、当日100%

If less than 7 days notice of cancellation is received your full payment of your golf camp fees is forfeited unless you can find a replacement.
No refunds given from the day of travel and after the program has begun In the event of a refund, the student is to bear all the transfer fee expenses.

また、現地のホテル手配発注後は、受け入れ機関などが定める取消手数料が別途必要となる場合があります。ご返金に要する振込手数料、為替手数料などは、お客様のご負担となります。
**旅行開始後の解除・払戻しお客様のご都合により途中で離団された場合は、お客様の権利放棄とみなし、一切の払戻しを致しません。
お客様の解除権
お客様は旅行契約が成立した後に以下の通りで定める取消料をお支払いいただくことにより、いつでも旅行契約を解除することができます。なお表でいう「取消日」とは、お客様が当社の営業日、営業時間に解除する旨をお申出いただいた時を基準とします。（お申出の期日により取消の額に差が生じることがありますので当社の営業日、営業時間、連絡先はお客様自身でも申込時点に必ずご確認願います。）
